St. Vincent's chosen for new program

St. Vincent 's Medical Center in Bridgeport was named by the American College of Cardiology as one of 35 hospitals nationwide pioneering a team approach to keeping patients healthy and at home following admission for a heart attack or heart failure. The hospitals are the first participants in the ACC Patient Navigator Program, which supports efforts to reduce unnecessary patient readmissions.
